The legal bit

By using the services of MotivatePT, you agree to be bound by the following terms and conditions. They are therefore important so please read and sign this form to acknowledge that you have read, understood and agreed to the following information, and to confirm you are aged 16 and over.


COMMITMENT
In order to maximise progress, it is important to follow program guidelines during both supervised and (if applicable) unsupervised training days in addition to any dietary recommendations given by Trainer. Client should follow the training program and instructions of Trainer to the best of their ability to maximise their results and better achieve their goals.

REGULARITY OF TRAINING
In order to maintain progress and as a courtesy to Trainer, it is important that Client commits to their personal training sessions on a regular basis (i.e. once a week as a minimum) and avoids taking regular or protracted breaks from training.

LENGTH OF SESSION
Personal training sessions will last for sixty (60) minutes unless otherwise agreed by both parties.

ATTIRE
Client must wear sensible and practical workout attire, including, but not limited to t-shirts, shorts, tights or tracksuit. Athletic shoes must be worn at all times and should be supportive and functional.

PUNCTUALITY
Client should be attired and ready to train at the time specified for their personal training session. Failure to be prepared to train does not entitle Client to a session longer than the scheduled appointment. If Client anticipates running late they should contact Trainer as soon as possible.

If Trainer is running late, Trainer will contact Client directly via telephone call or text message. Any time missed due to Trainer being late will be made up by Trainer, either at the end of the session if both Client and Trainer have time, or in a subsequent session when it is more convenient.

STOPPING EXERCISES
Client may refuse or stop any exercise for any reason at any time. Equally, Client may be directed to stop exercising by Trainer should Client display noticeable signs of distress. It is Client’s responsibility to notify Trainer immediately of any discomfort, pain, fatigue, nausea or other symptoms arising during or immediately after exercise, as well as, any and all other known physical limitations Client has or experiences so that Trainer may accommodate the Client and, if necessary alter their program accordingly.

PAYMENT POLICY
All personal training sessions must be paid in advance via bank transfer. Trainer will only be made available for appointments if payment is received prior to appointments taking place. MotivatePT reserves the right to cancel all booked appointments if payment is not received. Electronic invoices/receipts will be provided upon request.

REFUND & TRANSFER POLICY
All monies paid are non-refundable and cannot be transferred. Any unused sessions will expire within 6 months of their purchase date for blocks of 20 or 30 sessions. Unused sessions from a block of 10 will expire within 3 months. Unused sessions will no longer be valid after that time.

CANCELLATION POLICY
24 hours’ notice of cancellation is required for all booked appointments otherwise Client will forfeit the session and full payment will be incurred, regardless of the reason for cancellation.

DISCLAIMER
Client is required to fully disclose in the relevant questionnaires supplied all information medical or otherwise that is pertinent to the services provided.
